Appointments

We ask for a 2 day change or cancellation notice to avoid a cancellation fee. We understand that conflicts arise and we will do all we can to accommodate you should this occur. We in turn ask for your respect and consideration for our policies. Please arrive 15 minutes prior to your scheduled appointment to complete required forms (or register online now to accelerate the process). This will insure our schedule runs smoothly.

Forms of Payment

Payment and balances for all services is required at the time of service.

We accept Visa, MasterCard, American Express, Discover, Care Credit, personal checks and cash.

What To Bring

Please bring the following items to your appointment:
  1. Your completed New Patient Forms, if you’re visiting us for the first time.
  2. Your vision and medical insurance cards.
  3. Your current eyeglasses, including reading/computer or sunglasses.
  4. A list or bag of your current medications.
  5. A list or bag of your current eye drops.
  6. If you are a contact lens wearer, please wear the lenses to the appointment unless they are uncomfortable.
  7. A list or bag of your contact lens solutions and rewetting drops.
  8. Someone to drive you home. Some of our patients are given eye drops to dilate the pupils that may temporarily cause blurred vision. If you have had these eye drops at another examination, and they did not cause any blurred vision, you may drive yourself.
